The HFES-Potomac Chapter application form is now available on the web site! To submit your application or update, follow the steps below:

1. Click here to go to the application form.
2. Print the form on paper and fill it out.
3. Include a check or money order for the appropriate amount, (Payable to HFES-POC) and mail to:
